• Linking a survey to a form to collect a netid

  Surveys are anonymous, netids will not be supplied with results.  To collect netids while maintaining anonymity follow these steps:

  1. Create a form in the Form Builder that requires a login (something like one question, "enter your name to win a prize").
  2. Edit your survey and go to the Appearance/Screens tab.
  3. Edit the Thank You screen and add a link to the form created in step 1.

  After the user submits the survey they will get the Thank You screen that contains the link to the form.  Running a report on the form will supply the netids of those that took it.
